Esa50 Questionaire prsent at medical.

I have had four ATOS medicals since 2008. I have to say I have had no problems with the outcome from the last four medicals until my last medical making it my fifth which took place 8 days ago. After this 5th medical I was sent a letter telling me to attend a work focused interview at my local jobcentre as I had been moved from ESA support group to wrag.
I noticed at my previous four medicals the health professional always had to his disposure to his eyes my completed ESA50 questionnaire that was fully completed with detailed information about my condition.
However during my last medical I noticed something different! My Esa50 questionnaire was not present. It was no where to be seen. I did not see anywhere it and If I did not see it during the course of my medical. Does thit mean the health professional did not have the esa50 to his hand and was unable to read about my problems that where laid out in detail to help him make a clear decision about my claim.
During my recant medical. I feel the health professional had carried out my medical in haste and I felt under pressure to answer questions quickly under pressure and on the spot. I feel have had answered questions not as clearly I should have. Is the esa50 questionnaire always used during medicals?
I want to appeal about being placed into wrag.
What forms must I ask for before appealing against the decision to place me from the support group to wrag?

I don't think claimants can read too much into the ESA50 being present during the assessment or not, ATOS are perfectly capable of ignoring the contents in either circumstance!
You can appeal the Decision, the first stage to your doing this is to request a Mandatory Reconsideration, the process is explained in more detail in our ESA MR & Appeal guide

You can also request the documents used in the making of the Decision
ESA medical – what forms to ask for?
You can submit new evidence or clarification of your ESA50 during the MR stage and it is certainly worth asking on what basis you have been moved from the SG if your conditions have remained the same or are indeed worse, although you need to be aware that the ESA Descriptors and Regulations were updated in January 2013 and this could be the reason for the change.
You will continue to receive the WRAG rate while you appeal and will also be required to participate in Work Related Activity although you should not be forwarded to the Work Programme. It would be worth your phoning the JC+ advisor to tell them that your are challenging the Decision, it may be possible to defer the appointment or hold it over the phone.
