The DLA have used an untrue statement of facts sent by my Doctor to make their decision. I told them it was incorrect and not to use it and that a correct form would be completed using my paper medical notes by another doctor but they ignored this and went ahead and made a decision anyway. What can I do ?

Your only option is to appeal the Decision, a first stage to this is to request a Mandatory Reconsideration, you can do this over the phone but we recommend that you follow this up in writing.
You should make it clear that you will be submitting new evidence; the new report that you mention in your post, you should be given four weeks to do this.
I would also recommend that you request all of the documents used in the making of the Decision
