I called for my PIP claim about 5 weeks ago. After 3 weeks a received a letter saying I needed to send off two forms of ID before they would send a form out to me. As I didn;t want to post my passport etc I had to get the documents verified at Jobcentre Plus. Yesterday I received my form with the date it must be returned as apparently a month from the date of the letter.
The thing is, I managed to get the documents verified on 8th April, I posted the documents on 9th April, the earliest they could have received them would have been 10th April. Why did I then receive my form on 14th April with the attached letter stating that it was written/sent on 7th April? How can they have sent this letter before I even got my documents verified?!!This doesn't really give a lot of time to complete the form given that Easter also falls into this period.

I am afraid I can't comment on why the form was issued when the DWP were still waiting for information from you, anything we suggested would be a guess.
As to the form, you can request an extension, but unless you receive written confirmation of the change you should assume that the form needs to be returned by the original date, unfortunately the DWP take no account of public holidays when they set the dates for these things.
As you are new to the site you might want to have a look at the following FAQ which explain where everything is

In case you are not aware, your real name appears to be showing in the forum, if you want to change this then follow the instructions in the following FAQ
My full name is showing, how can I stop it?
