Hi, I receive DLA, awarded since 2012, but my condition has got worse. I was sent an application pack in September and started to fill it in, but then became to ill to continue. As this is not a new claim, do I need to only describe how my condition has got worse than previously reported; or should I complete the application in full? Can I ask for a new application pack as well, as I think I need to start over afresh. Can anyone advise?

Welcome to the forum,
You must fill in your claim pack as though you are starting from scratch otherwise you run the risk of losing your entitlement. But you should pay partcular attention to highlighting any deterioration in your condition particularly if you are looking for a potential increase. I am unsure as to how they will react to you asking for a new claim pack, hopefully others can advice who have done this.

In addition to Puccalove's excellent advice, I note that your username might be your real name. Many people prefer to use a pseudonym in order to maintain an element of anonymity (especially if their real name is more unusual).
