Some help with ESA claim for Fibro was created by sastek
Hi, I wonder if anyone who has fairly recently successfully claimed ESA for Fibro would be able to help me fill in my ESA50 form? I need someone who is able to get by without help. I am really interested in getting examples of what people have actually put on their forms rather than getting advice.
I can't get an appointment with CAB before the form is due back, but anyway I am really only interested in getting examples of what people have actually put on their forms rather than getting "advice".
First questions are general and they are
1) Did you provide any medical reports with your ESA50 form or did you just leave it for ATOS to contact your doctor
2) Did ATOS contact both your GP and the Rheumatologist/other specialist for reports.
The other questions I have are about what you put into the form about your condition and abilities.

You asked not to receive 'advice' but just to say Atos will not necessarily contact your GP and are highly unlikely to contact your specialist.

As a general rule we will not allow the sharing of benefit form content on the forum as it could bring the site into disrepute.
The ESA Claim guides have example answers which I would expect you to be able to adapt to your own situation.

I suggest you get copies of med reports from your GP, copy and add it to your ESA50 form.
Try and insist that your GP sends you to a specialist/consultant to obtain true diagnosis, get copys of those letters and send them in too.
They do take notice of your Consultant/Specialist reports more than just a GP acknowledgement that that is what they think is wrong with you.
Plus the fact with a proper diagnosis, your GP will take more notice of you and your medical probs.
