hi. 12 months ago I was placed in to the support group. around two weeks ago I received an atos reminder letter informing me that they have not received the medical questionnaire they sent out to me some weeks ago. the problem is I have not received the questionnaire from atos and I need to have it returned to them by the 31 dec 2013. today 7/1/14.

My question is regarding what I have said above and also why after only 12 months am I being asked to fill in the questionnaire again. is it normal after 12 months to be aske to fill in the medical questionnaire again in such a short time.

Hi im,

1 year is a little on the short side for the Support Group but DWP can actually reassess at any stage once 3 months has passed since the initial decision was made

How often will I be reviewed

You do need to get a form completed and submitted now to prevent your benefit being stopped. Although you would probably win an appeal on the grounds that the form did not arrive, it is a lot of stress going through the process. If you wish, you can download the PDF version of the form and use that rather than waiting for a hard copy to arrive

Using The PDF Version Of The ESA50 Questionnaire

Don't forget to make good use of our ESA guides

ESA claims and appeals

Hi im,

You might also want to e-mail WCA Customer Services which can be done from the link on this page :

www.atoshealthcare.com/claimants

You could inform them that even though you have received the reminder letter, that you did not receive the ESA50, and it appears that it has been lost in The Xmas post.

You could tell them that you intend to fill in and return The PDF version of The ESA50, and will return it to them ASAP.

If you don't contact ATOS, they may simply return your files to The DWP, and your benefits may be stopped for failure to return the ESA50.

thanks for the reply. as I said early. it was only 12 months ago when I last filled and returned the pdf version of the ESA50 form and I was placed strait into the support group once the examination had taken place at my local job centre. 12 months ago I completed and saved the pdf ESA50 form on my pc. I still have the completed pdf esa50 form saved on my pc. should i just make some slight changes to that form and then send it to atos or do i need to completely start from scatch by downloading a new form. i don't no if the form has changed within the last 12 months.

The form had a major update in January 2013 so you will need to download the current form, also the ESA Descriptors and Regulations were also updated at this time so you should have a look through the latest guides to make sure that you do not need to alter any of your answers.

Finally, we recommend that you also explain why your condition has not changed or is worse and that you have not been able to adapt to any of the limitations that result from your condition(s).

Lastly it worth making it clear that you are currently in the Support Group, so that anybody considering a different award will have to justify their comments as to why this is the case.
