The ESA50 is numbered using the WRAG descriptors not the Support Group ones, in fact two of the SG ones are not even referred to on the ESA50.
The WRAG descriptors are Physical; questions 1-10, and Mental; questions 11-17.
This is reflected in the two ESA guides in the Members Only area.

Thanks for your help Gordon. I am finding this so confusing.
I am going to fill in the form on my wife's behalf and sign it to confirm that I completed it for her. This might sound like a silly question but when I complete it do I complete as if it was her saying the things ie "I can't do such and such" or would I complete it by saying "my wife can't do such and such" as it is me filling out the form?

Hi SB,
It is fine to write in either "person", as long as you make it clear on the form which "person" you are writing in, and why.

Hi i posted my form on the 31st October last year and got a decision in the post yesterday 2nd February.

I attached previous med reports that dated back over 5 years - in my case my condition is progresive, so about 3 years ago I told my Consultant that she didn't need to send me copies of reviews that she sent to my GP. I did drop my consultant a line to advise that I had received a form that may have required me to attend an assessment, a copy of which I also sent with my ESA50. I can only assume that DWP/ATOS took this information on-board, as I was migrated across to ESA Support Group from IB with no medical assessment.
