I tried to complete the petition and not sure if i have signed it three times or not at all!

Certainly a vital cause, but it's going to need a lot more signatures.

I had a problem with apparently lost medical documents prior to a tribunal. They turned out to be 'upstairs on someones desk'.

Just a suggestion, the doctor will have been paid for the session so if you provide date, location, time DWP should be able to trace doctor's payment and possibly names of those attendint during that session.
