DLA form for GP to complete was created by Rachias
Hi there - thanks for your help on my last question.
I am caring/assisting a claimant for a renewal of his DLA. The claim form has been completed by a Benefits Adviser at our CAB. The adviser thought it would be a straightforward claim but that the GP may receive a form from DWP.
Is there anywhere on your website to see a copy of the form or at least what sort of questions GP will be asked.
My friend will try to arrange to see GP when the form (if any) gets to GP but he is a bit reluctant before knowing what the form asks. GP not that clued up on benefits especially mobility criteria which is quite critical to his claim.

Hi Rachias, I would get your friend to visit his/her GP. And go through all the difficulties they are having with mobility etc so that it is recorded in their medical records. Best Wishes

hi, I normally get a letter from my GP and send it in with the form. I normally get clear on the criteria either by getting the key areas from this site or ask Cab for a check list which I talk to my gp about. Generally if I send in a letter with the form they don't write to my GP as well which speeds up the renewal process.
