ESA 3. 10/13. Help needed

Are you claiming ESA?

This form is used to collect information about your income, assets and savings and any hours that your partner works, this is then used to help decide on a claimants eligibility for Income Related ESA.

If you are not claiming ESA then you should contact the DWP office that sent it to you, to find out why it has been sent.

Do you receive any other DWP benefits other than your DLA?

If you do then you should have received letters about those benefits from the DWP, the letters will have the phone number of the office that deals with your claims.
