I would send any SAR to the Benefits Office that usually deals with your benefits.
Although it will be forwarded on to The Data Controller for your region.
You should address it FAO The Data Controller, and write DPA 1998 SAR in front, top left corner of the envelope.

Separate issue but similar: Would it be a SAR I would need to send to ask about them losing my ESA50 & entire medical evidence as I have no idea when and if DWP actually ever received it from ATOS or is there another route to take?

Hi d,
A SAR is basically a request for copies of all personal information that the DWP hold on you within the time frames and parameters that you lay down.
If they have lost documentation, one would surmise that they will not be able to provide copy of it.
You could request written confirmation regarding any lost documentation under separate cover also, in the form of a complaint:
Complaining to The DWP
If you can prove that they have lost your personal information, then you can also complain to The ICO.

We sent my request for SOR off to Bristol as it is closer to us than either Leeds or the third location.
Obviously we are still waiting ..............
As we had help from our MP, in regards to waiting time and knowing which group I would eventually be in re ESA, I shall expect I shall ask him to intervene again if I have not had a reply from Bristol within 4 weeks of them receiving the said letter re SOR.
