Completed the ESA form had to the 20/11 to get it back in to them.Sent if off 25/10. Got a letter this morning from my GP to make an appointment to see him as he has a form to fill in for ESA.I asked the GP for a letter at the time,but he said no send in this patient summary,he highlighted the summary with all the conditions and tabs I am taking,so the medical staff must not take his word or my form and letter I sent in.On top of that I took a heart attack just a few days after sending in the form and damaged my heart I am told after tests done and have heart failure,so cant see my GP now until the end of the month for form filling once again.If only I had good health I would love to be out working and earning my money,and not spending my days in hospitals and filling in forms.

You should contact both ATOS, at the address you sent your ESA50 and the DWP office dealing with your claim to make them aware of your heart attack and the treatment you are now receiving, in addition if you are asked to attend an assessment, take a copy of the letter with you and ask them to read it and include it in your file.

Would your GP be willing to visit you at home to complete the form?
