Limited capability for work questionnaire was created by benson
I've been sent a E.S.A questionnaire as a result of moving from income support to ESA as i suffer from anxiety and depression for past 3 years,am at a loss as how to fill this out,am so nervous I don't know where to start,I would prefer to just have a medical,I had a home medical 3 years ago and passed this and was put in limited work catagory but after a visit from lady from job centre she said I should have been placed into support group,I was taken off ESA and put on income support,can anyone help with filling this questionnaire out,am finding it hard to put into words how am effected day to day

and FAQs

Make sure that you return the ESA50 form in good time, in order to ensure that your ESA continues to be paid.

N.B. It is very important that you fill in and return the ESA50 by the deadline that you have been given.
If you fail to do this, your benefit may cease, and it can be very difficult to get it reinstated.
If you are really struggling with The ESA50, then here is a list of potential sources of advice :

It's also asking for any medical proof,I sent what evidence I had from my g.p and psychologist to DLA as I had to renew my DLA claim 2 month ago,I was awarded another 5 years,will they hold it against me that I sent this evidence away to DLA and no longer have it

It's too late now to undo what is done. However, for future reference, we always recommend that claimants only ever send copies of medical reports etc to DWP (unless specifically asked otherwise). Is it possible that your GP will provide a letter for you? Some do, but an increasing number are refusing.
One thing you could consider is to ask to see your medical records and then take copies of whatever looks helpful. Another thing is that family and friends can also submit statements in support of your application.
