SANTA01 form

SANTA01 form was created by SCHSheff
HI,
I sent a SAR letter to the DWP based on the letter on in your appeals guide. They have sent a SANTA01 form back which is more detailed than the online one you direct people to on the gov.uk website.

In particular, there is the following:

"Part B: confirmation of appointeeship or power of attorney
We have no record of your appointeeship or power of attorney.
Please provide appropriate documentation and we will be happy to continue with your request."

There isn't a space for you to explain that this doesn't apply. I thought it obvious from my original letter I was enquiring for myself. It makes it sound as though if I don't provide such a record they will not release any information. They also ask for a current utility bill for proof of address.

Are they being very careful (Data Protection Act) OR is it just that the form is a rubbish design because it tries to cover everything OR am I being paranoid? Perhaps they can't understand why I'm asking for all this information when I was put in the SG.

I haven't heard of this before from any of our members.

However, on doing a quick search, this came up as the result of an FOI request regarding SANTA01 and a SAR :

If the santa01 form was sent from the data controller. could sheff use the email address to explain to the data controller that they have sent the wrong information, and to request the information he asked for in the first place

Thank you for the links you provided. Having ploughed through half of the first link all I can say think is good grief. I am beginning to get very confused about what I actually want. I wonder if my original letter muddled the issue. Part of that letter read as follows:

"This evidence should include:
- the ESA85a medical report form
- the LT54 form
- any medical evidence from health professionals such as my GP or consultant
- all other evidence considered by the decision maker in reaching their decision including the length of the prognosis of the support group award and the descriptors that were applied in coming to the award decision"

Are the last two points too vague?

I found a DWP Subject Access Request Guide online, although it dates back to April 2008. Optional paragraphs can be inserted into a SANTA01 for purposes of identification and clarification (and prevarication?). These include requests for a current utility bill and evidence of power of attorney. Looks like they just threw them all in.

Perhaps all I need (since I am in the SG this time) are the ESA85a and LT54 forms? My GP wrote a letter to go with my ESA50 and the prognosis and information about descriptors will appear on the forms anyway. Won't it? Am I over thinking this?

BTW, in response to Bud's reply, there wasn't an email address on the letter.
