DLA Change of Circumstances: lost orig claim pack

Update as promised. Warbreck House Blackpool came up trumps and sent me a copy of my original claim form. How simple life was back then! Such a short form, more a list of tick boxes. My medical notes were in transit at the time from one English region to another, but I got LRM and MRC with no problems. See my next post for the good news...
