Hi - I am a subscriber and I cannot seem to find the correct guide in the subscribers section to help me fill my form in.
None of the guides seem to correspond exactly to the forms I have.
I am asking for a supersession of my current claim and the forms they have sent me are :
DLA434 Sections 1,2 AND 2A
The forms are dated up the side border as "DLA434 Adult February 2007"
Any help would be appreciated. Many Thanks.
There aren't any guides specifically for the completion of form DLA434, which is the standard supersession request form. You will need to extract information from the appropriate DLA guides, which are pertinent to the sections of the form DLA434 which you are required to complete.
As you are already aware, the appropriate guides can be found in the section entitled DLA claims.
