DLA- Change in circumstances

I'm helping an eldery couple with their DLA claim. They both have chronic illnesses and have received DLA mobility rate for the past 8 years. However, the elderly lady's condition has now worsened and she is due for surgery soon. She used to receive the lower rate care component and higher rate mobility. She now needs alot of care as she can't move around easily; can't cook,clean,walk much etc. Could we claim the middle or higher rate care component now? Do we have to complete a new DLA form from scratch?
How do we go about telling DWP that her care needs have changed?
Any advice and tips would be much appreciated.

Write to the DLA Unit at Disability Living Allowance, Warbreck House, Warbreck Hill, Blackpool, Lancashire, FY2 0YE, or alternatively telephone them on 08457 123 456 informing them of the change of circumstances, and request the appropriate claim to enable an increase in the care component.
