I am helping my partner with her pip review AR1 form and have read the updated guidance from page 132 of the guide. We will put see attached sheets for the first 4 sections of each activity, if doing this do we need to tick the boxes or leave these blank, for example do you need help from another person yes/no. Thanks
If you need help from another person then tick yes. When adding pages make sure you number the pages and add your name and NI number to each page. I would write in the boxes please see page XX of YY.
YY being the total of extra pages.
