Side effects of taking medication was created by Connie
I take many medications that cause side effects, and I couldn't say which medication(s) I am adversely reacting to. Writing on the additional sheet of paper, can anyone suggest how best I could explain this?
I was considering writing all of the side effects I have and add that I'm unable to say which medication it is as it could be from all or most of them. Thank you.

Hi Connie,

Me personally, I would say exactly that. List the medications and what you're experiencing. It's up to them to have their medical professionals understand what is happening. It could be adverse reactions to 1 or all - you don't know.
