Uc50 form fill for mental health was created by Lisamaria
I, ve just received my uc50 form, very daunting, i need guidance how to fill it with the right words, i, m bi polar with all the affects that go with it, i,ve been migrated from income support to uc, so i have to start all over again, i haven.t worked for over 20years. Very stressed.

Hi Lisamaria,
Were you receiving IS as a carer? Have you had a DWP medical assessment ( PCA or WCA ) in the past?
Is UC currently asking you for Fit Notes? Sorry for all the questions !
With bipolar disorder you may experience the following symptoms which you could indicate on the form UC50----fatigue and loss of interest in caring for yourself during depressive episodes.
--during manic episodes you may be too easily distracted to cook
--during depressive periods you may have difficulty remembering to take your medication
--you may be too distracted to remember to wash and bathe or not have the motivation to do so
--during depressive episodes you may experience feelings of worthlessness that make socialising difficult
--during manic episodes you may make unwise decisions regarding important life events and money
--during manic or depressive phases you may not ensure that you are wearing clean clothes or you may sleep in your clothes
-- Because of delusions or tiredness you may find it very difficult to leave the home.

Thank you for getting back to me,, years ago i was on i.p,,think it was that, then moved to e. S. A,wrag, ,9 years ago my partner moved in,, we went on income support,, i had got pip for daily care,, high rate,, only assesment i have done is from P.I.P,,,,ha got carers to look after me., yes I have to send in sick notes,, they state i have bipolar, anxiety and known panic disorder.. The message you have given to me is so me with the bipolar disorder thank you..
I did sick notes years ago when on esa depression and agrophobia.,, was diagnosed 12 years ago with bipolar disorder.

Yes, I agree that you should find someone who could show you how to complete the UC50 form.
Have you tried your local Citizens Advice branch. Maybe you could phone them next week or else you could use Citizens Advice webchat, link here www.citizensadvice.org.uk/about-us/conta...us/web-chat-service/
As you are bipolar you could use the Mind Welfare Benefits Advice Line in the link here www.mind.org.uk/information-support/helplines/
There may be other Advice Agencies in your area who could help .

Hi,, i went through some old paper work today,, i found i did have a hca, i didn't have to attend face to face as the dr at the time wrote a letter to them,, can this make any difference to the form,, can they look at the assessment from then o tell theirs no change?

Thanks for searching through the paperwork. Can you please tell more about what you found such as the dates, what was the Benefit (involved maybe Incapacity Benefit) and was it a PCA ?
