Hi and thanks for your time. I am moving home next week to a flat suited to my disability. I have a change of circumstances pending with SSS(ADP). My condition has got worse. Because I am leaving to go to a different postcode area I will have to change doctors surgeries. My worry now is if I get a new doctor they won't know how my disabilities effect me. The doctor I have just now knows everything about me. I don't know what to do.
I'm afraid there is no way around this. The best I can suggest is that you make an appointment to see your new GP as soon as you are able to after moving so that you can brief them on your problems.
