www.gov.uk/government/publications/capab...r-work-questionnaire however, if you can't then they should send a replacement but they cannot extend the time for you to complete the form so continue writing out you answers so that all you have to do is copy in your answers when the new one arrives.
If it is just one or two pages, then you could print these pages and include them in the booklet as replacements.
