Thank you I will do just need to complete the box on what grounds is the Appeal?
this is what i amr eally stuck on plase help.
do I just say I don't agree with the award and will supply more evidence when I know more on their decision from the evidence they supply as there last letter to me completely ignores some of the info I provided and doesn't even comment on it!

The PIP MR & Appeal Submissions document has an example completed SSCS1, your reason for appeal could be as simple as "I believe that I have supplied sufficient information in and with my PIP2 to show that I qualify for an award", although I would recommend that you pick out a few specific examples.

What if I have additional info though but feel as though I need to see their evidence first? Do I wait to see their evidence and then supply mine?

Will this suffice?
I wish to appeal as I do not believe that the award I have been given is correct. Not all of the information that I have provided has been taken on board or used correctly and some of the information provided at the assessment has been misconstrued.
