Ideally you want your GP to right a letter confirming the limitations that you have documented on the PIP2 form and why you have them.
In practice there may be some Activities that they feel unable to comment, this will depend on how much information you have supplied to them in the past, typically these will be the more personal Activities such as washing and dressing, it will also be dependant on your conditions, for example if you have OCD then they may feel able to comment on these.
I would try and make the request a conversation as much as possible, as this may allow you to add additional content that your GP can use in the letter, but you should be cautious about leading them in what they should write as this can depreciate the value of the letter.
