The Centre for Health and Disability Assessments (CHDA) (Maximus) who took over the ESA WCA Contract from ATOS on 02/03/15. clarifying events, in that it was not you who refused to take part, etc.
This will cover you in case they try to blame you and state that you have failed to take part in two face to face assessments which could result in your file being returned to The DWP, and your ESA payments ceasing.
