For each activity you need to explain how you are restricted by your conditions, for some it may be possible to identify the cause of your limitation, as an example, perhaps you do not get dressed because of your depression, for other activities it may be more difficult to isolate a single condition in which case you might want to refer to the most likely candidates and finally for some activities you may have multiple limitations resulting from different conditions.
Again as an example; due to your depression you may not want to eat on a regular basis so you do not prepare and cook a meal without prompting, your Osteoporosis may mean that you can stand to prepare and cook the meal, and you ME might mean that attempting to cook causes you extreme exhaustion and perhaps problems with memory that mean that you often fail to complete the cooking process.
Don't be afraid of repeating your self from one question to another or of explaining each of your problems. A good way to tackle the form is to start with a one simple explanation for each activity and then either providing additional explanation or expanding one into more detail.

it says on the form please tick the box if you can move around and use steps with outdifficulty
then it says .....how far can you move safely and repeatedly on level ground without needing to stop I feel I should not tick any of the boxes I can not repeatedly walk 50 I can only do 40without stopping for a rest for 2mins its so confuse in
