Pip 2 form was created by cbrad
Have just printed off your July 15 guide for pip claims, is there a copy of the pip2 form anywhere that I can print off? I assume that the transfer from a dla lifetime award to pip is still going ahead from October 15. and I want to be as much prepared as I can be. Thank you.

There is an example form on the following link, however, you must not use this for your actual claim, each PIP2 form has a unique barcode and if this is missing then your form will be rejected which could lead to your claim being closed.

Transfers for indefinite awards have already started, they are being rolled out by postcode again, however, it is important to note that not everybody in a selected postcode will be assessed immediately, this will be done over the next couple of years, we assume that claimants are still being chosen at random.
