Someone else filling in my PIP application.

I am unable to write so my wife will be filling in my PIP form.

How should it be worded ?
Does she write as I dictate or write as if she's telling it from her point of view ?
as in :-
I can't open a tin or He can't open a tin ?

Thanks.

Rest of it seems easy enough after reading all the guides.

If she is using your words, then it is "I", if it is her words then it is "he", use only one version. If it is your words then you need to sign the form, it is your wife's then she does.

In either case explain how the form has been completed and why.
