I'm afraid there is no good answer to this!
It could be two years before you assessed which will depreciate any evidence that you collect now, however, you could also be called as soon as the next tranche of postcodes is announced.
I would start by talking to your GP about the issue and seeing how helpful they are likely to be and then go from there, many members have a file already prepared for the transfer that they update on a regular basis.
