i have received a letter to attend a W C A. on the 6 July ,my last esa 50 was 2 years ago
is this correct or do i need to ask for a new form thanks

Hi m,
This has been happening to a number of our members.
They have sent in an ESA50 a year or more ago, heard no more, then received a letter out of the blue from Maximus (CHDA) informing them of an appointment for a face to face without being given the opportunity to fill in a current ESA50.
You can challenge this !!
You should contact : The Centre for Health and Disability Assessments (CHDA) (Maximus) who took over the ESA WCA Contract from ATOS on 02/03/15.

Yes, we would recommend that you complete the ESA50 in detail.
