From the current PIP Claim guide, page 31

“Therapy” means therapy to be undertaken at home which is prescribed or recommended by a—
(a) registered –
(i) doctor;
(ii) nurse; or
(iii) pharmacist; or
(b) health professional regulated by the Health Professions Council.

Do you mean 1(e)?

e. Needs supervision or assistance to either prepare or cook a simple meal. 4 points.

You need to meet a Descriptor on the majority of days to qualify for points, in principle this means 51% of the time, but there will always be an issue of credibility where a claimants condition varies, so higher percentages are better than lower ones.
