ESA 50 pdf form

Given the recent concerns re ESA 50's not being sent out before f2f medicals I hope to have one prepared anyway just in case. My question is regarding the new ESA 50 issued in February this year? Is that the latest one available online in pdf form? Also, daft question probably but can you print this form off blank, as opposed to filling it in online? My tablet at home doesn't support the pdf format and I want to print a couple of blank copies off at the library. Unfortunately, the librarian couldn't really answer my query this morning, (she even opined that sending out ESA forms differs from council to council?) Thus suggesting local councils administer ESA, so as you can imagine I decided within five seconds not to treat her advice on downloading and printing them seriousl either. She wasn't sure anyway..
It looks like a blank one is printable but as my review isn't quite due yet, if it even happens this summer, I didn't want to download and print an old version of the ESA 50.

Yes, the latest version of The ESA50 is ESA50 02/15.

Using The PDF Version Of The ESA50 Questionnaire

You can print off a blank version.

You need Adobe Reader XI to save and type in interactively.

We have noted that the text boxes are smaller, and when typing in interactively, the printed text does not shrink to allow more text in the text boxes, as it used to.

Therefore, a lot more add-on A4 pages might be needed.
