ESA 50 interactive form

Hello

May I request some help please. I am trying to fill in the ESA 50 interactive
PDF form. However the print justs get smaller and smaller the more you print
the words. Does anyone know how to fix this??? baffled, hoping for your insight

I am afraid that this is one of the pitfalls in using the PDF ESA50.

My best advice would be to use the text box for as long as the text is of an acceptable size, then write a comment such as "continued at Page xx of xx" , then continue on a separate A4.

It is a good idea to print off a page of The PDF ESA50 to make sure it is legible and easy to read.

You don't want it so small that The Assessing HCP or DM has an excuse not to read it.
