Atos medical questionnaire form

I have been sent a Medical Questionnaire from Atos, stating that even though I have been placed on ESA group since 2012, but they want me to fill that out.
The head line of the form says (Limited capability for work questionaire) from job centre plus.
Could someone tell me if there is any guide on this site on how to fill this form to get the best result?

One of the mains elements of the Welfare Reform bill is that everyone claiming benefits will be reassessed periodically. You were allocated to the Support Group in 2012 so it sounds as if this is your reassessment. You must comply with this otherwise you will lose your ESA.
