DLA to PIP anxiety

You will be asked for
- your National Insurance number
- your address
- your date of birth
- your bank or building society details
- your telephone number
- the name of your doctor or health worker you want the DWP to contact
- details of any time you’ve spent out of the country
- details if you’re in a care home or hospital, or if you’ve recently been in a care home or a hospital
- whether you have a terminal illness.
Once you have completed the telephone interview you will be sent a copy of the completed
form to check, sign and return. If your completed form is not returned within the time limit
given in the accompanying letter your claim will be closed. For this reason, you should try to ensure that you get a certificate of posting when you return the form.
If you suffer from
- a mental health condition
- a behavioural condition
- a learning difficulty
- a developmental disorder
- memory problems
then you should be given extra time to return the form.
I'm guessing that the call should take 10-15 minutes at most.

I have merged today's post with your previous topic for continuity.
As you will see (from page 12) in our : PIP Claims Guide. (2nd Link on page)
The PIP 1 Form is used initially to capture basic information including your disabilities.
You could send copies of photos.
However, you should send all and any relevant evidence with The PIP 2 "How your Disability Affects You" Questionnaire, when you receive that.
As advised in our guide, ensure that the PIP 1 is returned by the deadline date given :
Sending Documents to the DWP or ATOS or Capita.
