The "benefit ceased 30/09/2014" should be in relation to your IB although I am a bit confused, as I am sure you are, as to why there are now three dates for your transfer!
The P45 is again in connection to your IB. IB was liable to tax, although it is possible that due to the length of your claim your IB was exempted, the P45 is a statement of any tax liability for the current Tax Year. Whether your IB was exempted or not Contribution Based ESA is liable to Tax and you should inform the DWP of the change.
Unless there is something fundamentally wrong with your claim, which be extremely unlikely if you have been receiving benefit for some time then the DWP will not change their Decision on your being placed in the SG.
