I received bad news this morning regarding my PIP application; they have turned me down. I am hopping mad. 
The DWP have been prompt, it is only three weeks since my medical, but their scoring is just completely wrong.
I suffer pretty severe anxiety and agoraphobia and find it very hard to leave the house, other than on a good day a couple of hundred yards to the nearest shops and café. I know this is not the worst case of the condition they will have come across but I was allocated only 4 points on the “Planning and following journeys”. This was, allegedly, because the descriptor that applies is: “You need prompting from another person to undertake a journey to avoid causing mental distress”.
This is patent nonsense. At no point on my application form, or in the Capita medical, did I mention the need for another person to help prompt me to go out of the house. It just is not the case. They could argue that I am not completely housebound, but that is nothing to do with which persons are or are not present. I have no idea where they have got this stuff from.
Obviously I plan to write immediately for a re-appraisal.
Just wondering if there is anything else I can do at this stage? Can I ask for a copy of the medical report?
Also, should I just send in your suggest standard letter for requesting an appraisal or should I flesh it out with the reason I think they are wrong?

I am sorry to hear that your PIP claim has been unsuccessful, you don't mention the Daily Living component, were you expecting to score points for this?
At the moment all we are aware of in terms of documents is the assessment report, there does not appear to be any forms number for it, you should be able to request this over the phone but we recommend following up the request in writing.
You do not need to give any reasons on your Mandatory Reconsideration request, the Decision Maker should phone you about the request and you can explain your reasons then. If you intend to make further submissions or to provide additional evidence then you should make them aware of this and the DM will allow you extra time for you to send the information in (4 weeks).

I also got 2 points for Daily Living for 'Engaging with other people face-to-face'. Again I was described as someone who could only 'engage with other people' when prompted by another person. This is even more ludicrous than the Mobility score.
