hello I have just been sent a dla renewal form , I was just wondering what type of evidence I should send I have a care plan from my cpn and my list of medication , a new med 3 form from gp which was done last Friday , a letter from gp stating I have schizo affective disorder and her opinion cannot work this letter was written for my Esa claim and I also a letter from my psychiatrist stating schizo affective disorder and reasons why I unable to work as these where written for my esa claim and giving resons on why I cannot work I was wondering if they where any good for dla as evidence ?

This is a full renewal of your DLA claim so you must treat this as if it was a new claim and provide all of the evidence that you have available.
Whilst I cannot say that the letters you have will not be of value, DLA looks at your Care and Mobility needs, your ability to work is not considered, except in these circumstances, it is perfectly possible for a DLA claimant to be working full time, providing that doing so is not in contradiction to any stated Care or Mobility needs.
I would recommend that you have a good look at our DLA Claim guides for a better understanding of the requirements.

Even though they have already provided letters for your ESA, there is no reason why your GP and psychiatrist should not provide new ones for your DLA if willing. Your care plan may well be of help.
It is really important that your renewal forms are returned to the DWP by the required date or you risk your claim being closed and your having no choice but to claim PIP.

yes I thought so and understand , unfortunately both my gp and my psychiatrist are away on holidays for the next month or so at the same time , but I do have my care plan my cpn nurse will fill in the part of the form stating what my illness and diabilites are and how they affect me it seemed to be enough last time I claimed in 2012

I mean cpn fills in the part , statement from someone who knows you. as advised last time on benefits and work I photocopied my last dla form , and although it is a new dla2014 the questions are exactly the same ,

um well it does state on both letters that I have frequent almost psychotic episodes dispite my level of olanzapine antisychoctic , hearing voices plus hulluctinations on a daily basis , which I guess does directly relate to care needs and my mobilily
