I'm a 48 year old lady who has cerebral palsy

I I have cerebral palsy hemiplegia from birth I'm 48 I'm being transferred from SDA TO ESA the ESA50 form is giving me nightmares , my condition will never improve now it affects my body on both sides can anybody help me with the right terminology for the form
