I am unable to see my GP, PIP Claim form was created by gillian palmer
Hi, Please have patience with me ,new to forum & nervous,I was sent a new claim form for PIP after having DLA high rate for both, i am not 65 until 11th sept 2014,the problem is i do not go to my GP regular i know i should, as i have many emotional problems to do with going out as well as limited mobility & i struggle daily with everything on my own, I do not think my GP really knows how difficult everything is for me & i was unable to get an appointment with him until after the date i have to return claim form( i have had extension) . I have no support from any organisation & the only family i have are worse off than myselfe,just would be grateful of any guidance or help please ,i am so afraid of the whole PIP thing & have no way to go forward.forms have to be sent in by 11th august 2014

Welcome to the forum :)

In case you are not aware, your real name appears to be showing in the forum, if you want to change this then follow the instructions in the following FAQ

It is crucial that you return you PIP forms by the date given to you by the DWP when they extended the period. Failing to do so will likely result in your PIP claim being closed and your current DLA claim four weeks later.

You can send further information to the DWP and the Assessment Provider later so don't worry about it not being with your PIP2.

Medical evidence is important but your GP is not always the best person to provide it, if they are not involved in your day to day care they may be unwilling or unable to comment on your daily care and mobility needs. That said, you should still discuss the situation with them to see what they may be able to do to help you, but make sure you have a good understanding of what they might be able to do, especially if they indicate that they may charge you for any letter or report.

Whether your GP helps you or not it might be worth your having a look at your medical records at your GP's practice, to see whether there is anything worth your printing and sending on.

Once the DWP have your PIP form they will forward it to an Assessment Provider to arrange a face to face assessment, you will be notified of this and once you have this address you can send on any further information, we recommend sending copies to both the DWP and the AP.

Write everything down, or get someone to type it all out for your GP. So say what you need help with, how your conditions affect you and what you have difficulties with, mention pain, breathlessness....... say how long it takes to get out of bed, how you get out, how long it takes to get dressed, what you wear to make this easier, if you need help with this, do you struggle with housework, is this done by someone else, how do you shop, how long it takes you, how long does it take to undress, get back to bed, how long washing yourself takes, if you can manage to get into the bath or not, can you do a simple meal yourself, or do you need help, are you in danger of burning yourself, do you forget there is something cooking...... can you get out on your own or do you need someone with you for assistance, to say, go to new places, ..... that way your doctor will have the notes on how your illness or disability affects you and this can be put in the doctor's report when the DWP request it.
