I only made my ticks in the form and wrote in the text box "see additional information Q15". Under the title Q15, I went through all the questions by dictating them to my husband.
It was 17 pages long, with the NI and PIP number on it and very important - page numbers like 15 of 17. I also made a list with the evidence, so nothing could get lost.
They did accept it without any problems.
As my claim took ages anyway, it was not really important, but Capita is not allowed to accept any documents/evidence at a home assessment. Everything has to go to DWP and has to be scanned into the system. Capita works with digital images rather than paper copies.
If you need more time like me, call them and ask for a 2 weeks extension. Make a note to whom you talked and at what date and refer to this when you sent the package in.
