Hi GJ,
A good way to approach this is to identify the medical conditions that you suffer from using written evidence of diagnoses and prescribed medication, etc.
Then concentrate on the limitations that you suffer as a result of the medical conditions.
You then need to tie in the resulting limitations that you suffer with the qualifying WRAG and/or SG descriptors.
It may help if you go through each question of the ESA50 whilst at the same time referring to the corresponding section/s of our ESA Claims Guides.
Remember, it is the limitations caused by your medical conditions that may or may not give you qualification for ESA, moreover than the medical condition/s themselves.
You might want to think about :
Using The PDF Version Of The ESA50 Questionnaire
Whilst referring to the guides on this page :
ESA Claims Guides
See also :
How long do I have to complete the ESA50?
ESA50 too big for the envelope
&
Sending Documents to the DWP or ATOS
To gain entry to The WRAG, you must score at least 15 points in total (cumulatively) across all/any of The WRAG descriptors :
WRAG (LCW)Schedule 2 Descriptors
To gain entry to The SG, you only have to fulfill any one of The SG Descriptors. :
SG (LCWRA)Schedule 3 Descriptors
As you will see, some of the WRAG descriptors (but not all) that score the maximum 15 points also correspond to some of The SG Descriptors.
I know it seems daunting, but try and follow our guides methodically, step by step.

You have already received a lot of detailed info but i thought I would chip in.
I would answer questions based on your current, not past, limitations. I would not rely on any subjectivity/sympathy of anyone reading the form. They are trained to objectively apply a strict test. Also this test is not strictly a fit for work test, although it is supposed to be, it is arguably not a 'real world' test. It is not a test of whether or not you could realistically hold down a job but whether a strict set of criteria apply to your case.
