Congratulations on The SG award, and thanks for letting us know.
It is not a definite that an ESA113 Questionnaire has been issued to your G.P.
Confirmation of whether one was issued may be available from your G.P. reception.
ESA113's are only issued in around a quarter of all ESA WCA's.
To be sure, you could make a formal request to The DWP for the ESA113.
You can also request all documentation used in coming to The SG decision.
