Hi can anyone confirm please that medical certificates or fit notes are not to be sent to Atos, but are to be sent to the jobcentre?
I sent my ESA 50 form back to Atos but am still waiting for a letter from my MS nurse and have just received one from my Neurologist.
Do I send these to the same Atos address that was on the (Atos) letter with the form?
This is very confusing
Also is there more than one ESA form? I have seen on here that some are 50 pages, but mine was 20
I'm off to A&E now, my dentist dropped the crown that I was having fitted on Monday and I swallowed it so have been advised to go to A&E due to chest pains.

Sick notes go to jobcentre dealing with your claim.
Medical evidence goes to the Atos office dealing with your assessment.
Yes there are different ESA forms. The ESA50 is a 'limited capability for work questionnaire' asking about your health conditions, other questionnaires access other aspects of your claim, such as, an ESA3 which is a means test.
Good luck at A and E, you might want to take a book!

You are correct that med certs should not be sent to ATOS, you should send them to DWP, the correct address will be at the top of any correspondence from them.
Medical evidence should be sent to ATOS at the same address as you sent the completed ESA50 (as far as I am aware there is only one version of this form). Many people like to send a copy to the DWP Decision Maker too.
Sending Documents to the DWP or ATOS
