Hi S,
Not heard of many if any of our members receiving these.
I know that you may be tempted to shred it, burn it, use it in the bathroom or throw it in the bin, but if you are not happy with the treatment that you have received, far better to fill it in truthfully and return it.
If you "bin it" ATOS will simply record this as you having no complaints.
It's up to you,
of course.
