Can you test our survey form?

Hi all,

We're going to be starting a long-running survey of people who have had PIP medicals next week. A test version of the survey is online and we'd be really grateful if anyone can spare the time to give it a try and let us know if they have any problems. You don't actually have to have had a PIP medicals, you can just make up the answers as this is only a test - we just want to know if the questions are clear and it works without doing anything confusing or unexpected.

Many thanks and you're quite right - it would make more sense to ask for the first 4 characters.

I'm having problems with the destination page on completion - I hope I'll have that sorted by next week.
