There was a discrepancy on the old version of the DLA guides after the forms were updated but this should have been corrected with the latest versions which are dated June 2013.

What is the question numbers for the following?
"Do you have physical problems that restricts your walking?"
"Do you need someone with you to guide or supervise you when walking outdoors in
unfamiliar places?"
"Do you usually have difficulty or do you need help getting
out of bed in the morning or getting into bed at night?"
